What do you need to apply for an ITIN?
Fill out and sign Form W-7.
Send your W-7 application along with your completed and signed tax return to the department in charge of issuing ITINs.
Documents you need to send with your ITIN application: original passport (if you do not have a passport, you need two original identity documents from your country, one of the documents must have a photo ID).
